legal capacity, hereby grants consent to LLC "ETP GPB," located at: 191186, Saint Petersburg, municipal
district Palace Square, 1-3 Malaya Konyushennaya Ulitsa, building A, room 19-N, for the processing of
personal data subject to the following terms:
